It’s very obvious that Ianto is going to be the one taking control in this situation since Jack is so far gone emotionally. While he’s eager to get closer to him it doesn’t have that same tinge of desperation and loneliness so he works gently to begin untangling and removing clothing from them both, moving carefully around Jack’s needy hands to get him that skin they both want - and in Jack’s case, need.
